The father of reflective practice, Donald Schön, considered reflection on action a defining characteristic of professional practice.(1) Although doctors have privately reflected on their practice since time immemorial, it has only been in the last 15 years or so that reflection on practice has gained wider credence, as reflective practice has become increasingly integral to the doctor’s professional role.

Making patient care safer is a key priority in medical education. (1,2) Emerging evidence suggests that preventable harm is an issue in general practice, including the specialty training environment.(3–5) We know that educators may unintentionally impact on patient safety in a range of ways including through failures of supervision, providing limited feedback on performance and letting poorly developed behaviours continue unchecked, while trainee doctors, are susceptible to error.

Symptom-based learning has become an integral part of the curriculum in most medical schools, although to varying degrees. This paper shows how a simple, visual model of illness can be used to facilitate this form of learning, as well as to help medical students more familiar with clerking patients on the wards and in hospital outpatient clinics adapt to consulting in general practice.

I would like to thank Kevork Hopayian for his recent letter(1) in which he described how he has used the Consultation Navigation Tool which I described in January 2012.2 He describes how he has included a box saying ‘gather all relevant diagnostic and prognostic data…’. This illustrates the versatility of the Consultation Navigation Tool, as in this example case of painful wrists, it is important for the doctor to find out about stiffness, swelling, etc.
